Follow These Steps To Get Started

Complete & Submit New Client Form

Please select and complete the new client form below that applies to you based on how your services are billed: Auto Insurance/Worker’s Compensation or Health Insurance/Private Pay.

Check Payment Options

Check our payment options to confirm that we accept your insurance. If you do not have insurance, we also offer private pay options. Please attach and submit any supporting documentation with your form.

That's All!

Once you submit the new client form, you will be directed to a page that states, “Thank You! Your Submission Has Been Received.”

After you submit the new client form, someone from WBC Counseling will contact you within 48 business hours. If you do not hear from us, please do not hesitate to contact our office to confirm that we received your form.

Auto Insurance or Worker's Comp

If Auto Insurance or Worker’s Compensation covers your services, please complete this form.

Health Insurance or Private Pay

If Health Insurance or Private Pay covers your services, please complete this form.

Insurances We Accept

Auto Insurance Claims

Services related to motor vehicle accidents may be covered through auto insurance, depending on claim approval and required documentation.

Worker's Compensation Claims

Services related to workplace injuries may be covered through workers’ compensation insurance, depending on claim approval and required documentation.

Blue Care Network (BCN)

We are in network with Blue Care Network, coverage as well as out-of-pocket costs vary by plan.

Blue Cross Blue Shield Traditional

We are in network with Blue Cross Blue Shield Traditional, benefits vary by individual policy.

Veterans Affairs Community Care Network (VACCN - Optum)

We accept VA Community Care referrals through Optum, prior authorization is required before services begin.

Medicaid

Through our internship program, we accept Medicaid clients, coverage is dependent on eligibility and required authorization.

Insurance Disclaimer

If you don’t see your insurance listed, please don’t let that stop you from reaching out, as we also offer private pay options with sliding scale fees.
